Completely fake due to this: The so-called 'surgeon' is apparently too stupid to perform a simple lobotomy to make the girl docile/mindless, and instead chooses to go through all that work.

Edit: Lobotomies really are extremely simple. They can even be done with ice picks. Pretty much just damage at the prefrontal cortex until their personality is gone.
