Clearly this is the fat man's birthday and he is chowing down on his favorite meal of spaghetti, clams, shrimp with lots of garlic bread. The lazy susan originally had a pizza on it which the guy in the back ate most of. The guy in the back is checking to see if he shit himself in the mirror from eating all that pizza - not realizing that a photo was about to be taken by the clown hired for the party. A hooker (quite possibly Britney Spears) was invited as he had few friends and didn't want an all sausage festival going down.

                        I think I figured it out.

                        The guy in the back is in cahoots with the cameraman. Notices the sly eye contact he's making with the camera. They're pranking the fat dude, who apparently paid to have his birthday catered and paid strippers and/or hookers to attend.

                        They know he's been eating so much spaghetti while guzzling down beer (notice the half-full beer glass in the front of the shot) so fast that he's about to make himself puke. Prankster A conspires with Prankster B that his farts smell so terrible that it makes fat guy gag, and that he should let out a massive fart while Prankster B photographs fat guy's reaction.

                        The result was a resounding success. Prankster A lets down his pants and lets it rip, resulting in a fart so terrible that fat guy starts puking up his spaghetti and beer all over himself while Prankster B takes the photograph at the exact right moment, while the lady of dubious virtue looks on with surprise, but not shock.
