His jaw (or specifically his upper jaw, called the maxilla) is narrow, hence the maxillary expander seen on the roof of his mouth (palate). His radiograph would just look crowded because of the overlap of the 2 rows of teeth. P.s. They are not 'ingrown' teeth, they are his normal teeth that have erupted (come out) palatally (behind the normal arch basically) and the orthodontist is attempting to get more space in the maxilla with the expander whilst moving some teeth around to allow space for those misplaced teeth. ELI5: Some of his teeth grew behind his other teeth because his jaw wasn't big enough so the dentist is stretching his upper jaw with a metal device to make room. At the same time, the dentist is also "pushing" the teeth that grew behind forward as the jaw gets bigger.

The xray would only show his teeth as crowded together because an xray is flat, like a 2d movie compared to a 3d movie.

To clarify, the spring on the roof of his mouth is an arch expander as his upper jaw is too small. The second row of teeth is where a pair of his adult teeth are in line with a pair of his baby teeth which have yet to be lost. The ortho should have ordered the extraction of the primary teeth first. The quad helix expander isn't going to work properly if the arms aren't resting on the lingual surfaces of the bicuspids. From the looks of it, they're resting on the palatal gingiva, which besides being ineffective, is just going to cause the patient discomfort.

If he has a lisp, it probably is the extra teeth. Braces don't cause lisps. Personally, I had them twice without a lisp and can't really think of anyone who had a braces-causes lisp. If people with braces have lisps, it would generally be because of overbites/underbites, etc., not the braces themselves.

Baby teeth aren't attached to the jaw like adult teeth, thus there would be no difference in his jaw structure. Although based on the extender attached to the roof of his mouth, his jaw and face structure is small as it is.

If you really want to. Is it really worth it? It's not like everyone is looking in your mouth. I wouldn't have if I didn't need to. My extra tooth was directly in front of another, so when it was removed, that tooth had no gums over it. I had to get the gums stitched in front of my tooth. Definitely not worth it if not necessary.

I have pretty much the same thing. My baby teeth were 100% then when my older teeth grew out they were fine too, I had a perfect row of teeth. Then it's almost like I grew extra teeth, honestly, I have more teeth that most people. I've had a few pulled but the gum problems they cause really isn't worth it just cause.

What I imagine they're trying to do is push the two teeth with the metal bands on them apart while dragging the premolars into their proper position. They may also be trying to make room for two of the four "innie" teeth and eventually schedule the the other two to be extracted. The small, pointy, ground down nature of the two inner teeth on either side of the middle two makes me think they're either supernumaries or primary teeth and would thus be the most likely candidates to be extracted.

The archwire and braces I imagine are in place to keep the rest of them in line while they spread apart so he doesn't end up with rotated, tipped teeth all over the place.
