Here in NZ "giving head" is entirely interchangeable between genders, and I could be entirely wrong but I think it's the same in Australia.

WRONG. Giving head refers the method of giving pleasure. i.e. using ones head to pleasure the other. That is why it can be used for either men or women.

Even on girls it's been used interchangeably since at least the 90's. Most of the time it was one of the other terms like eating out, or muff diving, or eating beavage...actually that last one might have just been my one friend.

i've always thought it meant the big head not the small head(guy). Usually its like guys talking about how a girl gave him "head" if it meant that its usually a girl giving it to the guy. If it means a guy can't give it to a girl then it would mean the girl has something a guy doesn't and "head" would mean other than a human head when the only part the girl gave the guy would be a human head. I don't know of any other thing a girl has that would also be named head.

I actually didn't quite know what the correct slang for this was and I'm in my twenties, female, and far from being a virgin. I've heard it referred to as 'giving head' by several different guys I've slept with.
