I legit had the same kinda thing happen. When I was really young I did the same thing, burnt my lip while blowing out the candle. I did get the flame to go out though....but I covered my lips with hot wax.

I've got it on VHS somewhere. The scene sees me blowing out the candle and then the lights are black since the flame is gone and all you hear is a blood curdling scream of a 6 year old.

The dark subtext is that that guy has been molesting the kid, and the kid thought that's what he was supposed to do. That's why he keeps his hand over the child's mouth, he is pretty much in shock that this happened in public and doesn't know how they will react. But his instinct is to keep the child's supple mouth safe from the fire so it will service him again.

Also look at the child's reaction. He thought he was going to be rewarded like he normally is when he puts the candle in his mouth, instead he was restrained and scolded. That is why he is confused and sad.
