They will eat living flesh but they prefer rotting / dead. They also secret[e] chemicals which may combat bacterial infection.

The perception that people have that they only eat rotting flesh is a bit misleading as humans and animals can be "infected" with maggots in live healthy tissue.

Actually, no. You would think that between not eating live flesh and dying they would choose live flesh, but they will actually not eat any good flesh, they won't.

Maggot treatment is a recognized and safe way to remove dead flesh from a living patient, the maggots can accomplish this task with much higher precision and safety than a surgeon, they really are useful little things.

Prior to going into nursing, I worked for a year as an ER tech. One night we had the fire department give us a heads-up that they were bringing in a patient that smelled worse than many of the several-day-old dead bodies they had recovered. We took it with a grain of salt, and continued on as normal.

When the guy arrived, the gravity of that warning became apparent. The patient's leg looked much like the one pictured above, covered completely in maggots. 'Failure to thrive' was one of the many diagnoses, IIRC. Anyhow, the smell was so overpoweringly bad that despite rooming him alone and packing the bottom of the door with towels on either side, most people in the ER were left holding their noses. Some requested to leave AMA (against medical advice) in an effort to escape the nauseating smell.

Poor guy lived in his own home and had family nearby. They just stopped visiting him, and he stopped caring.

I love how people against socialized health care seem to think that since hospitals can't refuse treatment based on ability to pay, it means the care people like this guy need is free. It's not. The hospital just has to recoup the costs elsewhere, which is why your insurance premiums keep going up. Medicare and medicaid reimbursement is crap - in some cases we're lucky if the basic supply costs are covered. So non-medicare patients get billed at a higher rate. It's how the system works. Or doesn't work, as the case may be.

This isn't so much upsetting as it is sad. This looks like a wound that has been there for a long time. Obviously he can't afford it. So he's been walking around with this horrible thing going on for quite sometime. This poor guy... The smell must be horrendous. I'm normally squeamish about maggots. The guy looks so calm about it. The sad thing is that he might have to get his leg amputated. He won't be able to afford a prosthetic, just crutches. I hope that he will get the best medical attention he can get.

You don't understand how medicine works, nor about the quality of the American healthcare. It's unethical for doctors to provide less healthcare to someone just because of his economic status. In the US, hospitals are also obliged to treat anyone in an emergency situation, regardless of ability to pay. America also has some of the best hospitals in the world.

While in Iraq, we extensively covered the topic on which limb you would prefer to lose, and the consensus was leg below the knee. The prosthetics are amazing and you could possibly function to a near-normal capacity.

Obviously this is a homeless man, so this probably doesn't apply to him since he obviously can't afford prosthetics. That fucking sucks!

I like to think I'm a good doctor though not in EM or surgery so take my opinion with some salt.

The only useful approach to this will be surgical, all the dead flesh has to be cut away and the remaining healthy tissue around the wound will likely be closed secondarily, meaning the gap will fill in with new tissue over the course of months. At best they can cut away some superficial necrosis and save the leg; if it's too deep and involves bone he will most likely lose a large part of the leg. This is in addition to antibiotics and such.

Your Surgery consult has arrived. Yes we would take this guy to the operating room, debride all the maggots and dead tissue and survey the situation to see if the limb can be salvaged. The thing to remember is that bone, nerves, blood vessels and tendons need soft tissue coverage via muscles, fat, or skin grafts. If there is enough tissue left to cover or a reasonable flap can be brought over to cover it, the limb can be salvaged. If there is too much exposed bone, tendon, blood vessels, nerves, or if distal circulation is compromised, we would proceed to an amputation. This type of neglect is rare but still is seen.

Whomever took/shared this photo is a douchebag.

Taking a photo of the leg is somewhat excusable, as a case could be made that it could serve as a teaching moment, but not only does this show the guy's whole face but it shows him having his clothes cut from him, and he deserves some dignity.

If anyone did this in the ER where I work, they'd be fired immediately.

But is this true of all maggots? I know there's sucha thing as maggot therapy that uses specially bred disinfected maggots, but it doesn't follow that all wild maggots are beneficial in the same way. Are maggots on a wound always a good thing?
