Yes, those are maggots. I've taken dressings that become contaminated with fly larvae before, you take off the bandage and they just wiggle at you. You anesthetize them with ether, then debride (scrape) them off. On a small wound, they actually keep it clean underneath, but you have to wait for the skin to regranulate and heal.

It's only an issue if you use maggots from flies other than the Blow Flies. The reason being that other maggots would feed on living tissue. The maggots from Blow Flies would feed on the dead tissue.
I had to do a project on Maggot Therapy last year. So I had to dig through my fair share of maggot pictures.

You know what? You're right. I didn't see the blood and maggots all over the inside of his pants. Once doctors apply maggots, they would use a packaged dressing that would already have maggots in them and they would attach that dressing to the leg.

This link is a medical video. The homeless man isn't the same as the one in the picture above, and the maggots mostly just cover his foot. It ends with him getting a shower, getting his leg cleaned off, and a nice shot of his oddly pristine foot. You get some amusing reaction shots from the doctor, but the man in the video had the good kind of maggots that actually, according to the doctor, helped keep the wound clean.

Most counties have Community Action Agencies that deal with this sort of stuff. They may not be able to directly provide aid or funds or anything for people, but they have access to this.
Local churches and government agencies also typically have all of this information available for people in case they need it.

Have you ever tried to attend one? Infinitely long lines and shit hours or the clinic, in poorly set up, and often poorly cleaned, waiting rooms where one is more likely to catch something else. On top of that you must also be able to prove that you need free medical services. Yeah, one of those situations where "you get what you pay for", in the worst way possible.

I've gone to a couple 'free' clinics. You wait in line for several hours to get in and be told they can't help you because the doctor didn't come in that morning. They give you a flyer for other clinics so you wait till they are open to find out they are only seeing 7 people that day and you were #11.

Not in rural areas there aren't. There are places where the closest hospital is three hours away and only one doctor's office to serve several towns. Free clinics are great for the areas they serve, but there are areas in the U.S. where I would not want to become sick or injured - insured or not.

I have NEVER heard of any place in modern-day USA where there are no hospitals for hours and hours.
There is generally at least one hospital (often more) within an hour of all locations ('cept if you're in rural Alaska or something). They might not have all the same capabilities/staff/facilities, but just like police stations and fire departments have to be everywhere, hospitals also have to be located so that they serve the communities around them, within reason.
Please tell me of one place in the US where the nearest hospital is 3 hours away (180 miles = 60mph x 3hrs).
Ok, three hours may be a stretch for the continental U.S., but there are rural places like Vermont (Groton), California (Yountville), South Dakota, etc. where hospitals are a drive of two hours or longer, usually due to the terrain. I'm not saying it's common, but very rural areas don't even have hospitals or doctors, never mind free clinics.
Free clinics, perhaps. And even Urgent Care clinics aren't everywhere.
But I think the government gives breaks to hospitals that operate in geographical areas where there is a need, simply for emergency reasons.
20 years ago I'd have 100% agreed with you, but all I'm saying is that nowadays, every American is pretty much within an hour or less of a hospital with ER capability.

This only works with sterilized maggots. My wife is the charge nurse of an ICU and she's done this hundreds of times (leeches, too). They have a particular species trucked in from California, and you're supposed to kill them in acid/alcohol after a few hours because they can acquire pathogens.

Honestly, that is a likely outcome, but it would be days before that decision would be made. Taking a person's limb is only an easy decision if the person's life is at stake. From the look of this video, amputation is inevitable, not to mention antibiotic therapy for probable sepsis. Also, these cases are likely to develop staph infections such as necrotising fasciitis.

You'd need to give that leg a good cleaning out and suss out what nerve and tissue damage he has sustained. If it's as bad as it looks then, ya, he most likely had it amputated. However if it's still viable, they may have considered negative pressure wound therapy that is often used in pressure ulcers.

You misunderstood what cosworth99 was saying. He would have been turned away with a smaller injury, which eventually led up to the condition he's in now, which will likely require much more expensive amputation. So instead of treating a smaller wound for less money, now the hospital is going to have to do a much more expensive procedure. Hence "no idea how to spend resources wisely".
A hospital won't turn away someone with a maggot infested leg.
That's not the point. The point was that the wound that lead to the state he's in now would not have been treated by a hospital.
Say he fell and his calf met with some glass, leaving a big gash. If he went into the hospital with the gash a few hours after it happened, they would've turned him away or treated him only if they billed him. Obviously, he can't pay the bills (assuming he's homeless).
Instead, the wound has to get to this point to be treated by the hospital for free, as it's to the point of being life-threatening.
Stitching up the original wound and giving him some anti-biotics would've cost... what $200 at most? Whereas cosworth99 says that amputating his leg will be $100,000.
God, this country and its practices are good at disappointing me horribly.
He wouldn't get turned away from an Emergency Room.
He may not get all the subsequent medications and follow-up visits provided for w/o money/coverage, but the ER can't legally turn away any person in a legitimate emergency.

The funny thing about this situation is that having maggots all over his wounded leg is better than not having them there. Maggots eat the dead flesh, keeping the wound clean and stopping it from becoming necrotic. Without our good friend the fly, this man very well could have been dead from an infection.

That is completely incorrect. Some maggots will feed only on dead tissue, some only on live tissue, and some on live or dead tissue. Blow fly maggots are just the ones that are most commonly used.
They most definitely saved his leg. Had they not been there it would probably have been amputated.

That actually might be Charlie..the well-known alcoholic of Bruceton, TN. He was found in the local pig slaughter house, slumped over in a chair with shit running down to his shoes and maggots eating his leg. I've known Charlie since I was a wee child. He would drink a whole case of Bud Light within an hour and pass out in our yard. His speech was always incoherent like Boomhauer from King of the Hill but somehow my grandpa could always understand him. My grandpa once asked him if he knew who Shakespeare was and he responded with, "mumble No dad gummit mumble dang it." Poor Charlie is probably dead now. No one has seen him around for about a year now.
