"Realistic" is not realistic. If you've shot a handgun, you know that you actually need to look down the sights. Doom is actually the closest to reality.

When you fire a gun you don't concentrate on your arm you concentrate on the gun. You have to take into consideration that the shooter is aiming, he is "zoned in" not starring at his forearm.

Depending on circumstance. One might bend the elbow to shorten arm distance in close quarters, advancing around corners, and you wouldn't want to lock the elbow at full stretch for an aimed shot. Keeping the joint bent a slight bit is better for the transference of recoil. But my real issue is that there are never (aside from CS) any left-handed player models.

If you see "realistic" when you try to shoot (and both eyes are open), then you're aligned with your non-dominant eye. Either learn to focus with the eye that's on you dominant hand's side, shoot across your center of body (bring forearm over a minute amount and slightly "break" your wrist), or close one eye (really not recommended from an awareness perspective).
