Not fake. This is a thyroid-induced problem called mammaritisis, which involves the nipples to swell to such large sizes. While not dangerous it is most certainly not a comfortable problem to have. It can be fixed with medication, but many choose to instead "milk" the nipples by squeezing out the puss as the nipples become inflamed. Many cultures (such as those in the far east) believe the puss from such inflamed nipples can aid fertility and both men and women (typically couples together) will take part in rituals, going as far as 'breast feeding' directly from the source. The goal is to manage to swallow and keep down the foul smelling (and tasting) puss - puking, though not that uncommon, is considered a bad omen.
