In the Bolivia episode he stood between two cars as one of them was towing the other and held the tow strap as he told Hammond to drive in a direction that would effectively see him become a pivot between the two cars.

Actually, I just took my firearms safety course a few weeks ago, and you do in fact look down most gun barrels for possible obstructions/rust/etc AFTER you've verified that both the magazine and the chamber are free of cartridges.
But if you can check from the breech, then do it.

Always treating the weapon as if it's loaded, even when confirmed otherwise, is probably the most effective way at preventing human error from causing an accidental discharge.
There's also things like people freaking out when you're waving an empty handgun around - no one really wants to think "is it loaded?" when the barrel moves past them. Treating it as if it were loaded even if the breech is open and the magazine's on the table just displays to everyone around you you're competent enough to handle it.
TL;DR: It can't hurt to live by that mantra, so why bother "confirming otherwise"?

Well, treating my bp rifle as loaded all the time is ridiculous. If I put a cap in it and fire it off and see smoke come out the other end, I know it's not loaded. At that point I'm pretty much gonna have to put parts of my body in front of the barrel to load it, breaking your written in stone rule, and I'm fine with that because I confirmed it personally, for me, that there's no charge or bullet in the barrel.

People make mistakes. It's possible, however unlikely, that you could become confused, and mistakenly believe that the weapon is unloaded. By assuming that the weapon is always loaded, you avoid this uncertainty.

Here's the problem in my mind. You tell somebody who has half a brain but knows nothing about guns "assume a gun is always loaded" they're gonna think "well that's not technically possible, there are times when a gun is unloaded". But then, you drill it in to them that there are no exceptions, there is no way to confirm that a gun is unloaded, and they're gonna think "well that's just ridiculous and doesn't make sense". It invites needless confusion and in some cases (and I've seen this) cause the person to pretty much disregard all the other safety information you're trying to tell them (other really important stuff that they needed to learn). Have you ever been in a classroom and had a teacher say "always" or "never" and you just know that's not possible, so you kind of stopped listening? It's like that.

The rule is always treat the gun as loaded even when confirmed otherwise. Everyone has had a time when they are 100% convinced of something but it turned out they were wrong. If you are 100% convinced your gun is unloaded, you should rightly be perceived as dangerous.
