That's the L5/S1 vertebrae. Where lumbar meets the sacrum. "Colon" while funny is below that and since it's not a bone, won't show up on Xray. Speaking of which, you need an MRI to see nerve damage. And if a disc has slipped, we need a lateral Xray, not the one you posted. (FTFY)

I have the lateral xray too, but it contains 100% less ostriches, and wasn't funny enough to post. It feels like a pinched nerve, the pain is localized and intense (not shooting), aspirin and other anti-inflammatory relieve pain quickly, and the xray was taken as a baseline because I've never had one before. Was quick, in-office, and was done to be preventive just in case. Agreed the doc said it probably won't help much based on the symptoms. Prescribed Meloxicam and Tramadol.

Honestly, I have no idea what the appropriate imaging study is to rule out ostrich. I imagine X-Ray would be sufficient since ostriches are kind of bony, but this isn't something I've read about.

When we do spine surgery we fuse two or more vertebrae. We gut the disc, put in spacer to restore height. Then we fire 50mm screws right in the Ostrich's eyes (surgeons call them owl eyes, though. RLY). Then we connect the tops of the screws with rods and lock the whole construct down with caps. Then your body grows bone and fuses the segment. IANAD but It looks like there might be a fracture on the left side (right side of the image) of the L4 or L5 pedicle. This could cause some problems resulting from a narrowing of the foramen.

Most of the colon is actually above this level (ascending, transverse, and descending). The sigmoid colon is possibly part of what you're seeing overlying these structures, but part of it is also ileal loops of small bowel.

It's impossible to tell with this limited, coned down view, but there is some suggestion of partial lumbarization of the superior most sacral vertebrae, which could correlate with low back pain.

Stenosis is the narrowing of the spinal canal. An MRI gives you "slices" of the area. It would be better to see from that.

I assumed that foraminal stenosis implied narrowing of the foramen, where I'm guessing a lot of the pinching happens.

I understand that having slices is much more useful, but I'm guessing that MRI's are often less available the XRays. Also, from my understanding the resolution in the current clinical protocol for MRI is not quite good enough to see individual nerves/tracts decently. Am I wrong? If I am, do you have any examples to the contrary, I'd be curious.

MRI can see amazing detail, with slice thickness down to less than a millimeter. It depends on the power (magnet and processing/protocol) of the machine.

I believe that this is possible, and even done in certain circles (experiments, research, ...)

But in going to much-sub-milimiter resolution (say 0.5mm) and milimiter thickness, wayy to many problems arise (artifacts, etc). I don't know much about this area directly, but it is what I am told/have read. That's why I don't believe it exists in clinical use. My guess is the slice resolution in clinical scans is 1.3mm+ and thickness around 2. Can someone with acquisition experience confirm/correct me?

with Gd contrast in MRI seeing tracts like the cauda equina (while still nerve "bundles" is still fairly impressive) are quite easy to see but since this modality is quite lengthy to wait for CT is modality of choice since Myelography is something of a lost art (at least in canada)

There's research in doing this visualization without contrast - see this for example. Still in research stages, but seems impressive. There, I think, you have a real advantage in using MRI.

Myelography using general duty Xray is rapidly being phased out in most if not all hospitals (strictly speaking in Manitoba, Canada) since CT has a shorter wait list than MRI yet far more detail then General Duty Xray. I wasn't refering to MRI or CT in that comment. Thank you for the interesting study though will definitely have to read through it :D!

Yeah the foramen describes the area where the chord resides. So when that canal gets smaller, it impinges the chord. The MRI's I look at you can see nerve roots. However, that's not the problem, usually. It's something impinging on the nerve. We can see extruded discs and degenerative dics on the MRI. That's what you need for diagnosis. I am in several spine surgeries a week. I'm a manufacturer's rep for spinal implants. Look at the spaces between the vertebrae. They all should be similar in size. If one is noticably smaller you might have degen disc disease. What's you height, weight and age? That would tell me a lot.

You can see nerve roots? What protocol are they using? From my understanding in current clinical scans it's pretty hard to see the actual roots - if you do they are generally very bad resolution, and, like you said, diagnosis is based on everything else around.

What do you mean regarding the height,weight,age? Was that a question that you mean would tell you alot of your patients? Or are you asking me for some random reason :)

I work in LA. But for any surgery with implantable devices there a rep in the room. (Hips, Knees, Spine) Exceptions are like small fractures, general ER type stuff. We bring all the tools and implants and make sure things are assembled and ready when the doc needs it. Answer tech questions, offer suggestions. Act like his third eye.

What they look for in the nerve root is to see if they will impede access to a site. There's nothing much you can glean from their image. Not defined like you say.

We operate on a lot of unhealthy people. Over 40, under 70 and over weight describes a lot of the patients. Stay lean and do your sit up. Gotta protect your core.

The bird like appearance of the spine is actually something we were taught to use in medical school for looking for spinal abnormalities. The vertebrae normally look like an owl with 2 open eyes but if there's is cancer or other abnormality present on one side it can make it look like a winking owl-
