Not really WTF material. A bit disturbing at first glance, but nothing really wrong or horrible here. A couple has one of those underwater births, with the husband in the water to help deliver. Their other children want to be a part of the moment that their new family member is born, so they get in too, probably in swimming trunks. Besides the bloody water, there's not much to this.

I'd never jump in the water with my parents while mom's giving birth, but that doesn't mean it's wrong or WTFy.
