Necrotic tissue. Basically the untreated diabetes this guy has stopped the blood circulation in his foot. Without blood bringing oxygen and other essential materials to the cells, they die, and unlike the regular planned death of cells, they don't break down into reconsumable materials for the others cells, but just remain as slightly shrunken, dead cells. All the black areas are beyond saving and will be removed, at least with the technology we have today.
