Once the buckle is past the twisted section, you are done. The twist should be undone and the buckle should be facing the correct direction once again.

It actually happens a lot over time. Its usually to do with the seat belt fibers. It's not that hard to sort. It's not the SO's fault either.

Actually hers is about 6 months newer and mine has about 8k more miles on it. I AM CAREFUL WITH MY SEAT BELTS. They are life saving devices!

It's actually pretty easy to fix. Fold the belt over on itself and then pull (hard) the clip over the fold.

I spend a high percentage of car rides when in the back using the fold and pull method to fix seatbelt clips that have been rotated improperly. I hate when the strap is rotated improperly.

Okay, I will be that guy. I just had this issue and ended up un-bolting the belt from the floor-board and fixing it that way. I tried to fix it like in the instructions and it wasn't happening.

Nice third row seatbelt in your Honda pilot, it you actually completely unhook both the shoulder and lap belt from the seat before putting them down then this won't happen. That's why the belt comes from the ceiling and has a slot for both hooks. Just a helpful tip from a Honda pilot owner.
