The image might be altered, but I'm pretty sure this is the professional bodybuilder Frank McGrath. Look him up and you'll see he has some ridiculous forearms. I'm actually positive this is him. I remember reading somewhere that he doesn't work on his forearms anymore because they are already like 18 inche!

if you pinch off or occlude veins, they can back up and form hemorrhoids, varices, stuff like that. Usually it's see in the rectum(funny), abdomen(scary) or esophagus(quick death). It can be seen in the scrotum where it presents as "a bag of worms". I've never seen it in an arm before, but I'm fairly sure that's what this is.

Probably deep vein thrombosis (blood clot) from muscle compression and the superficial veins are the only way to get blood back to the heart.
