Fat. But not just due to the existence of fat. It makes it harder to preserve the tissues.

'Formaldehyde' (or, more likely, some other similar substance) absorbs into the tissues (muscle, skin, etc.) of the thinner person much easier. In the larger person, the fat (triglycerides) seeps out and dilutes the 'formaldehyde', inserting fat into the other tissues while also limiting the 'formaldehyde' absorption.

In reality, the larger person is probably more similar to an un-preserved human specimen, in terms of color & consistency of tissues.

Source: I took Gross Anatomy, and had a 250 lb woman as my cadaver. The rest of the class had relatively "healthy" (thin) specimen.

To the people noting the differences in the bony anatomy: The discrepancies are mainly due to the level that this particular slice shows. You'll note that the humeri on both patients are only half there, you can only see one bone of the forearm, part of the skinny girl's femur isn't visible, and neither patient has a visible fibula. If the whole set of images were available to scan through, you'd likely see a normal skeleton.

I think that was probably the point of the images. Only so much of your body weight is 'necessary' (aka bones, muscles, organs, things necessary to keep you alive). So past a certain weight, your mass will start to become fattier, and expand with fat and excess.

Just had a gyn scan today, and the doc pointed some out dark areas on the screen as intestinal gas. She even showed me the liquid in my bladder. Kind of surreal.

Only way to keep it off is through permanent changes. And weight loss business does not want you to succeed because they lose a customer. Do stay away from bad science and strange fad diets. The universal law is literally "calories in calories out." Protein is superior in hunger supression and is also worth the least calories, so it is best to prioritize meat in a weight loss diet.

This is an MRI scan, so it's a cross sectional image.

The section of the woman on the right is taken from further back in the body, so you can see the spinal cord in the neck. Whereas the woman on the left is a more central body image where you can see the back of her eye sockets and central nasal cavity.

Pretty sure those are eyes. I don't think these scans are at the EXACT same section depth (relative to skin surface) of each subject. Notice how on the smaller woman, you can see cervical vertebrae and brainstem. On the larger woman, all you see is muscle tissue. This indicates a shallower section depth on the larger woman. Furthermore, you can see the nasal cavity of the larger woman, again indicating that it is a shallow section.

The smaller spots in the middle of the brains are ventricles which produce cerebrospinal fluid. I'm not rock solid on my cross-sectional anatomy of the brain but I think the larger spots under the brain are cisternae("caverns") where cerebrospinal fluid flows/is stored. The scan seems to be rendered such that fluid appears black. The reason they appear different is likely due to the larger patient having her head raised further off the table and possibly angled back more due to having more tissue/fat on her back and head.

The Omentum is an organ that serves as temporary storage of fat in the body, is basically a small sack that hangs below the stomach. In fit people, the Omentum is small and webbed, like a couple of nylon socks hanging off the stomach. In obese people, the Omentum is bloated and forces other organs out of the way. It puts pressure on your diaphram, crushing your lungs and making it harder to breathe. That is why the obese woman has smaller lungs than the smaller woman, giving her heart the appearance of being larger. Her heart also seems to have a layer of fat around it (which, needless to say, is bad).

The 120 lb woman has her ankles in anatomically correct, 'flat', position. While the 250 lb woman has her ankles plantar flexed 'extended'. You can determine this by the position of the bilateral talus bones (bone directly beneath the distal tibial bones).

Her legs are farther back from the slice plane. Notice how her femur disappears behind the thigh muscles above the knee. So you are seeing a slice of her tibia that is narrower because it is farther forward.

It might be because her body is bigger than the 120 pound woman that her legs hang back away from the center plane of her body.

You're correct. The heavier woman is being shown as a more ventral (closer to the chest) slice, and the lighter woman is a more dorsal (closer to the back) slice. So, you're just getting the "front" of the leg bones in the large lady. This is mostly evident to me by the position in the brain, spine, and heart (since I do neuroimaging research and that's what I know best!).

It is because the section isn't taken at exactly the same point in the two images. So the slice through the tibia on the thin person was closer to the middle of the bone than on the fat person

It is not healthy that fat people have larger hearts. It's the body's way of adapting to needing to supply more blood. When the heart does this, it is prone to heart-attacks and cardiovascular diseases. Being obese puts tremendous strain on the heart, so no, it's not healthy.

If it's a CT scan, dark areas are less dense. You can usually tell if a person has gas by a black area in the stomach. (disclaimer: I am not a radiologist). If dark area was shit, then this person inhaled a ton of it into her lungs.

I suck at reading MRI's though.

No they don't, the only combined scanners are PET+CT and more recently PET+MR. The parts that make a CT scanner work can't survive inside the MRI's magnetic field.

This is pretty crazy. I am a guy, but I went on a fairly radical diet with Kaiser (totally monitored, they gave me all the food supplements I was to eat, I had to take biweekly blood tests, weekly doctor visits/ nutritional education class, the whole 9 yards) because in June I weighed 346 lb. Now I weigh 236 lb, and I am still very much overweight, I think still technically in the obese catagory, but very much better off (and looking to drop down to 200 at least still). I used to wear size 48 waist pants and XXL shirts, and I really filled them up. Now I fit very comfortably into size 38 waist pants and XL shirts. We did body evaluations last weeks (they take size measurements at the beginning of the class. For men, distance around your nipple line, distance around your belly button line, distance around your right thigh, distance around your right calf, and distance from your arm pit to the tip of your middle finger) and my chest and belly measurements decreased by 30 cm, my thigh decreased 20 cm, my claf decreased by 4 cm, and my arm actually increased by 4 cm (my arm didn't get longer, my arm just became less enveloped in my torso). It's a pretty crazy thing though because of the 108 lb I have lost the doctor said about 65-70 was fat and the rest was what they call lean body mass. She said normally losing over 40 lb of lean body mass is very bad because usually that is muscle, but the normal losses for the program are about 52 lb on average so my 108 is very much a special case. She did say I burned a little muscle, but most of the lean body mass I lost was like infrastructure that your body builds when you get that enormous (extra water storage, extra blood vessels, extra crap to do stuff with all the fat) and she said those were good losses. I went from 25% body fat to 14%, 9% is considered healthy. I can tell you the difference is absolutely staggering. This picture does a lot of justice to what that kind of obesity really is. You become something that is less recognizably human. Fortunately it can be dealt with. It doesn't take a lot of effort, but it does take a stiff upper lip and a lot of time. My insane 108 lb in 4 months is not really a realistic goal or rate of weight loss; I was losing something like an average of 8 lb a week through this problem, healthy weight loss is like 2-3 per week. I should have lost this weight at that rate for about 3 years, but my primary care physician said I could be prediabetic in 6 months (this was in May, I had just turned 23) and I really didn't have the 3 years to play chicken with getting a chronic disease like that, especially since I would be doing it on my own and he frankly didn't trust my resolve to stick to a diet and exercise plan for 3 years, so he recommended this special program for me. So I short cut about 2-3 years of good dieting with this to get where I am, and now the real test starts where I see if I can continue losing or if I can even just tread water. I will say I am pretty optimistic because now I can do stuff like run and hike, things I always liked doing in high school, but things I have found that I could no longer do after college. It's good to be like a human being again.

You are absolutely correct, I misread my thing. It shows % body fat broken up into section, 14% goal, and the rest excess. Before I had 24% excess (so 38% total) and now I'm down to 9% excess (so 23%). I need to change that.

These are artificially colored coronal T1 MRI images.

Everything that is made of fat looks bright on T1 - bone marrow, subcutaneous fat, fat in the brain, mesenteric fat.

Parts of the patient that show up black are water, air and cortical bone like the lungs, GI tract and bones.

The major differences between these people appears to be the amount of subcutaneous and mesenteric fat.

Whoever colored it in didn't make the pelvis bone look like the rest of the bones on the patient on the left of the image.

There's a slightly different slice selection between the two.

I've gone from 170+ lbs to about 145lbs within the past few years. I've dropped about 15 in the past 6 months alone. I feel so much better now than I did heavier and looking at this(though I was nowhere close to 250) makes me feel so much better.

what makes you say this? Hips and knees look even. I see perhaps a little shift in the neck area, but this could be from how the person was placed on the scanner, pads used for blocking. Or perhaps they do have a little something going on in the neck. I myself have disk issues in my neck and upper back that is visable on a MRI/CAT, but not scoliosis.

In all fairness a lot of people use that phrase to describe build or shape than as an actual excuse implying they literally have big bones(of course a lot of people do that too). I have a niece who is perfectly healthy and in really good physical condition, and not fat but clearly not skinny and just feels like she weighs more than she should. Family uses the term just to describe her appearance since she isn't "fat".

I have been around big boned girls. They are not fat (unless actually fat). These girls are often close to 6 feet tall if not 6 foot and have broader shoulders/chest when compared to an average shaped female.

I sort of imagine seeing them in football pads. Granted one of these ladies could knock me out in a second. So i keep my pipsqueek mouth shut around them.

as a large, stocky man with ~ 12% body fat let me tell you the truth. While one may have larger or denser bones, the real "problem" is greater muscle mass. I'm about as wide as a doorway (at the shoulders) and weigh a bit over 200 lbs.

Actually, the upper sockets of the femurs are clearly much larger in the obese woman than the thinner one. Also, this isn't a perfect vertical slice of the bones - the composite image shows all the slices at the same plane of orientation, which wouldn't capture that. As a result, the bones in the legs and arms appear thin or even non-existent because they were slightly out of the vertical plane being shown. So it is possible that the bones on the heavier woman are much thicker overall, and given the size of the sockets, they probably are.

Actually, the knee joints of the heavier woman seem thicker, but the leg bones seem thinner.
