No you don't. While there are probably some people who repost intentionally in the hopes of getting the kind of upvotes a previous poster got, others do it accidentally, and others do it intentionally, but not for karma, but because they think maybe not enough people saw it the last time.

In this case, it was posted three months ago and scored 1,003, but if it's new to you and you liked it, why not upvote it? There's no rule against reposts.

Yes and no. While it comes as no surprise an average male brain will go straight to tits, a lot of people see a repost, go to a former post, find out what the top comment was, and then copy/paste it in the hopes of reaping karma rewards.
